Our Work
At Health Share of Oregon, we work with the communities we serve to help everyone get better care
We know that many health problems come from unfair systems that have hurt some groups more than others. By listening to community members and learning from their lived experiences, we can design programs and policies that make care easier to access for all people.
-
When we say “community,” we mean everyone connected to Health Share, including:
Members who receive care through the Oregon Health Plan (OHP)
Tribes, representing our Tribal and Native communities
Governance groups such as our Community Advisory Council (CAC) and Clinical Advisory Panel
Community based organizations (CBOs) and nonprofits who partner with us
These groups help us understand what makes communities strong, what they need, and where we must do better. We are committed to working alongside communities so that we can:
Learn what members need
Build trust between health systems and community groups
Improve programs and policies
Move toward health equity for everyone
-
Health Share goes beyond state requirements.
Our goal is to share decision-making power and reduce harm in systems so that all communities have a real voice in their health care.
Much of our work is guided by the 2025-2029 Community Health Improvement Plan (CHP), which focuses on three areas: equitable access to care, prevention, and social health.
These are our priorities because of what we learned through our Community Health Assessments. These assessments give communities a chance to share with us what matters most to them and what health challenges they face.
