2026-2029 Strategic Plan
Health Share of Oregon’s 2026-2029 Strategic Plan provides a roadmap for advancing health, well-being, and equity across Clackamas, Multnomah, and Washington counties. Through collaboration with health plans, providers, and community partners, the plan outlines key priorities to strengthen care, support members, address system challenges, and build a more connected and sustainable health system for the future.
Our Priorities

Support people in staying covered, understanding their benefits, and remaining connected to care by coordinating clearly across the Collaborative, clarifying partner roles, and directing resources where they can have the greatest impact.
Objectives:
Lead coverage stability efforts across the Collaborative.
Serve as a central hub for outreach and navigation support where it matters most.
Prepare proactively for required coverage and program transitions.

Engage in a very targeted collaborative-wide strategies to better manage the cost and quality. This includes both work to manage populations in a way that optimizes care at lower cost and works to ensure that rates are sustainable. We want to innovate, learn, and effectively advocate for solution to budget challenges that make sense for member outcomes and the system.
Objectives:
Advance a shared regional approach for people with high-acuity behavioral health needs.
Strengthen prevention, early intervention, and mental health promotion for youth and families.
Sustain strong quality incentive performance while reducing disparities.

With the transition of Behavioral Health to a shared-risk benefit, we need to develop the infrastructure for shared decision making, budget projections, and implement strategies to balance cost, quality, and access. We also need to develop similar skills in our role as the final adjudicator of appeals in developing network standards. We want members to have as similar of experience of care as possible across our collaborative.
Objectives:
Strengthen the sustainability and reliability of physical health care and related services.
Strengthen the sustainability and reliability of behavioral health care and related services.
Align physical health and behavioral health care coordination across the Collaborative for members with complex needs.
Progress shared clinical network standards for priority care.
Align approaches to social health supports as policy and financing evolve.

Ensure that Health Share is positioned well for the next procurement—operating as efficiently as a collaborative as possible and meeting all expectations of contract and compliance requirements. We want a solid and efficient foundation as possible to head into the next procurement.

Maintain strong regulatory, contract, and financial oversight performance.
Advance shared advocacy and proactive system leadership grounded in equity and sustainability.
Foster shared accountability and governance to achieve more sustainable outcomes.
Mission, Vision, and Values
Health Share’s Board of Directors recently approved a new Mission, Vision, and Values that will help guide our work and partnerships in the years ahead. At a time when communities, partners, and health systems are facing significant challenges, these statements keep us grounded in the principles that unite and clarify how we show up together.
The new statements reflect months of engagement with our Board, Community Advisory Council (CAC), Member Organizations, and staff. Equity remains our anchor. Our new lead value, Equity First, reflects what we heard clearly: Equity should shape not only what we aspire to achieve, but also how we make decisions, navigate tradeoffs, and prioritize resources.
These statements set the foundation for our 2026-2029 Strategic Plan (coming soon!) and the work ahead including supporting members through changes associated with H.R.1 while continuing to improve health and advance equity across our communities.

With the leadership of our Chief Health Equity and Impact Officer, Phyusin Myint, and the partnership of our Community Advisory Council and Board of Directors, Health Share is proud to share updated, Board-approved language affirming our commitment to health equity. At a time when shifts in our social and political landscape have raised questions about the future of this work, Health Share remains steadfast and unwavering in our commitment to advancing health equity.
This updated language honors Health Share's ongoing health equity journey and builds on our previous commitments. It also lays the groundwork for a set of specific, measurable actions that will drive meaningful progress toward our broader health equity goals.

Health Share acknowledges that inequitable health outcomes are a deep and lasting consequence of structural and pervasive racism against marginalized populations, in particular, communities of color. Health Share believes a health system that serves everyone depends on strong partnerships across communities and sectors. Together, we can deliver responsive, sustainable, high-quality care, honor members experiences, and promote fairness in leadership and decision-making.
Achieving health equity requires taking sustained action to address avoidable differences in health and care. This includes addressing the conditions that shape health – such as racism and the lasting impact of inequitable policies and practices.
