Ride to Care
Helping your patients, together
The care you give your patients goes beyond the office—and can include helping them get to their appointments. Ride to Care makes it easy for you to help Health Share patients with their trip needs.
Trips are available 24 hours a day, 7 days a week, 365 days a year.
Traditional Health Worker
A Traditional Health Worker is a non-clinical provider type who, through shared life experience, can support members in getting services and care that support their health and well-being. THWs can help with:
Bridging communities and the health systems that serve them
Helping navigate health care and community resources
Advocating for health plan members
Supporting adherence to care and treatment
Wraparound Care
The Wraparound Process
Wraparound is a team-based planning and problem-solving process to help youth and their families struggling with emotional, mental and behavioral problems.
The process begins by identifying the family's strengths and needs and then creating a plan to meet these needs. Wraparound brings together a team of people involved in the youth and family's life. A family's team might include friends, relatives, school teachers, counselors, Child Welfare caseworkers and/or Juvenile Justice counselors. Care coordinators help all of these people work together and coordinate the activities and services that take place in the least restrictive setting possible. Most services are community-based.
Health Related Social Needs (HRSN)
Health Related Social Needs (HRSNs)
Health Related Social Needs (HRSNs) are social and economic needs that impact an individual's ability to maintain health and well-being. Through Oregon's 1115 Medicaid waiver, the state will be able to cover new HRSN services for eligible Oregon Health Plan (OHP) members.
