Every Step
Family-centered support through foster care and beyond
Every Step is a community of clinics. These clinics support the health and well-being of kids in foster care. Children in the foster care system face extreme challenges. So do the families that care for them. Our teams of experts make sure each child gets the highest quality care. This includes ongoing and holistic care they need, both during their time in foster care and beyond.
Project Nurture
A Center of Excellence model integrating maternity care and addiction treatment.
Project Nurture provides prenatal care, inpatient maternity care, and postpartum care, integrated with outpatient addiction treatment and comprehensive case management and advocacy services. Participants are educated about policies regarding DHS Child Welfare reporting, and we believe this transparency facilitates a trusting relationship with providers and allows us to be effective partners and advocates for families.
Housing Pilot
The Housing Benefit Pilot (HBP) launched in May 2022.
Through this two-year project, we enrolled 505 individuals who were at risk of or currently experiencing homelessness. The program was designed to support individuals at vulnerable transition points in their lives by connecting them with community-based Housing Navigators as well as resources such as rental assistance and move-in support. Vulnerable transition times included leaving substance use residential care, reentering the community from the corrections system, and exiting the foster care system.
Regional High Acuity Behavioral Health Projects
Health Share is investing in programs that help people with complex behavioral health needs get better, more connected care.
Many of these members live with substance use disorders (SUD) and experience frequent hospital or emergency department (ED) visits. These projects focus on improving care coordination, expanding access to treatment, supporting housing needs, and helping people stay safer and healthier in their communities.
Autism Assessment Capacity Project
The Autism Assessment Capacity Project (AACP)
began in 2021 to investigate and address the issues contributing to the limited autism diagnostic capacity in Oregon. It is a collaboration between leaders at Health Share of Oregon, OHSU Institute on Development and Disability, Providence Children’s Development Institute, Legacy Randall’s Development & Rehabilitation Program, CareOregon, and Children’s Health Alliance and was seed-funded by Health Share of Oregon starting in 2023.
